We are committed to providing comprehensive obstetric and gynecological care for everyone. Because insurance policies vary, we can only estimate your portion and coverage in good faith but can not guarantee coverage. We always encourage patients to contact their insurance company to confirm how services will be processed.
Payment of estimated portion and co-pays are required at the time services are rendered.
We require the following before we can provide you care and treatment:
- Current insurance information
- Referral if it is required by your insurance plan
- Co-payment or payment for non-covered services
Additionally, we will submit all insurance claims for you. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within 30 days.
A service charge may apply to the following:
- Co-payment not received within 24 hours of services.
- Re-filing of insurance if incomplete or incorrect info given a time of appointment
- Returned checks
Termination of care may also result from noncompliance of recommended care including missed or multiple rescheduled appointments.
